Course Registration Letter

Dear Dr. Bill Vicars,

I would like to register for your "American Sign Language Course."
I understand that the cost is $483 per course plus I may have to pay the cost of a blank disk or other video media and the postage to send that video to you for evaluation, (or perhaps I can simply upload the video to a server).  I understand that in addition to the online assignments I must complete a proctored final examination as well as a video project each at a level of 70% or better in order to pass this program.   I have access to a video camera and the Internet.

I understand that your transcript is for continuing education units (CEUs) and/or documentation purposes only. I understand that you do not  award college credit.  If I'm working on an accredited degree I should approach my local college, school district, or employer before enrolling in this program to determine the transferability of your CEUs.  I understand that ASL University is not claiming to prepare me for employment nor to become certified in any ASL-related field.

I understand that successfully completing this program will require approximately 60 hours per course of study, practice, and testing, plus a few hours to do a research paper (one per course), and a few hours taking care of procedural items (like submitting my video).

I understand that I'm entitled to a full refund for the first 3 days after enrollment.  After which there is no refund as long as you provide me with an appropriate curriculum, reasonable access to a qualified ASL instructor for questions for up to a semester (16 weeks), reasonable access to the ASL University website (Lifeprint.com or equivalent), and an evaluation of my final exam and my video project. I understand I'm not guaranteed to pass. My grade will be based on how well I do in my studies based on the professional experience of a qualified ASL instructor.  I am at least 18 years old and/or submission of payment will indicate permission from my parent.

I would like my course enrollment period begin immediately upon receipt of payment.
How this course is being paid for:
[If you are paying by credit card via paypal -- indicate whose name the payment is under, if you are paying via check, then indicate who's name is on the check.  If you local school district is going to pay for the course then indicate the name of the person or district whose name is on the the payment.]
